Osteomyelitis and multiple osteonecrosis of the knee in a patient with steroid treatment

Abstract

|
Osteonecrosis or avacular necrosis (AVN) is a well known complication in patients who are treated with corticosteroid, and it usually occurs in the femoral head. However, multifocal oteonecrosis and osteomyelitis associated with steroid treatment are not common. We describe here a 32 year-old woman who presented with 2 months of progressive unilateral knee pain, and she had been receiving oral prednisolone for the treatment of focal segmental glomerulosclerosis for 8 months. Magnetic resonance imaging showed multiple bony infarcts in the bilateral knees. Three phase bone scan showed the features of active osteomyelitis in the left proximal tibia. A computed tomography guided biopsy of the left tibia confirmed the presence of active osteomyelitis. The pathogenesis, diagnosis, natural history and treatment of this disease are briefly discussed with an emphasis on the importance of early diagnosis.(Korean J Med 71:S1084-S1089, 2006)
